Looks like a shadowbox was used and later tweaked with deformers. Is it so?

Yes, I tried it right after seeing your deviation. It works especially well with new Move Topological brush. I found it very fun to use: first make cluster of random blobs in shadowbox, second - edit each of the blobs directly with Move topo. Good for creating spraying blood:evileyes:

I'm glad to inspire you!!...but try the deformation options better...twist,inflate, bend...you can get faster results..Hehe...nice idea!!!

Yes. I am very familiar with Zbrush. I wouldn't call myself a pro or anything, but I have done a few things in it.

Of course!!..you know it very well, great works!!...I made it using shadowbox with a custom alpha, them deforming the mesh with the deformation tools

It is a new modeling feature of Zbrush 4.0 called Shadowbox. It works pretty simple: you draw the projections of detail on the sides of the cube, and 3d mesh is generated in real time based on this projections. Check the tutorial videos on the Pixlogic site. In this case autor generated random sponge like shape in Shadowbox and tweaked it with the Move brush.

Aye, I am familiar with the new feature however I've never been able to get up the courage to try shadowbox. ^__^; Looks interesting for sure.
